Prerequisites:
- Your user account has Group Membership for a Group with the Permissions for Setup - Insurance Plans Screen Full Control.
Gathering materials/resources:
- Printer setup has been completed within your printer's manufacturer instructions
Step-by-step instructions:
- Access the Setup > Insurance > Insurance Plans dropdown menu.
- Select your plan using the Search feature on the left.
- Make sure the Plan tab is selected and then press the Edit button.
- Change the field for Forms to Print to your desired selection (ex. ADA 2024 Form or HCFA 02/12 PrePrinted*) and then press Save to confirm.
- From now on, claims on this insurance plan will come in the format expecting to be printed with the newly selected form.

Select the plan you wish to edit, then select the Plan tab, and press Edit. Change the Form to Print field as desired.

- NOTE: some forms, like HCFA 02/12 PrePrinted, will assume that you are printing the claim onto a pre-printed form. This claim form generates text with no boxes or descriptions for what the information being printed is. Please see the linked article above for more.
