Up to four (4) dental insurance plans may be attached to a patient record.
Click the word “Patient”
Click the words “Insurance Information” and “Fourth Dental” options from the drop-down list
Search for the patient’s insurance plan:
· Based on the parameter stated in the “Search For” field, type the first word of the desired search text in the “Search Text” field (a minimum of two characters must be entered, without spaces)
· The “Search In” field should read “All Insurance Plans”
· Click the Search button
A listing of matches for the search parameters is returned in a pop-up box for selection; double-click the desired insurance plan
The Carrier, Employer, and Plan Information fields will be auto-populated according to the saved plan information (Setup–Insurance–Insurance Plans)
Confirm the Subscriber Information, and indicate the appropriate “Patient Relationship to Subscriber”
Indicate the appropriate “Patient Rel to Sub”
Indicate the appropriate “Qua. Sub Rel to Prim. Sub”
Click the Save button
When the patient’s third dental insurance plan is attached, a hyperlink indicating 4th-Dental appears in the “Dental Insurance” header.
