In the Medical History section, we sometimes need to write in information and alerts that are not listed. How can we allow for space for this on the questionnaire?

There are several ways to mark information in the Medical Questionnaire and the Dental Questionnaire screens:

·   Checkmark box – allows the box to include or remove a checkmark in the box to indicate the generic presence or absence of the condition

·   Yes / No / Blank selection fields – allows the indication of a specific “yes” or “no” to indicate the presence or absence of the condition; also allows that the field shows blank to indicate “does not apply” or “patient did not answer”

·   Text fields – allows for a small amount of text to be entered

·   Memo fields – allows for a large amount of text to be entered, limited to 1024 characters
