In a situation like this, you should enter the provider twice: one for each state. (setup/providers/provider setup/add new provider)
Be sure to have some sort of indication in the provider's name so you know which provider should be used in which state. Example:

Dr John Smith (NY)
Each provider needs to have the "Works At" tab show only the correct office(s) (setup/providers):

Dr John Smith (PA) ... needs to have only the PA location(s) selected

Dr John Smith (NY) ... needs to have only the NY location(s) selected
Be sure to attach the correct provider to the correct office as the billing provider (setup/offices)

Dr John Smith (PA) ... needs to be the billing provider for the PA office(s)

Dr John Smith (NY) ... needs to be the billing provider for the NY office(s)
The individual state provider should be assigned to only that state's location. By doing so, the staff members are not likely to select the wrong state provider when they're making appointments and charging transactions to the patient. If the PA provider is assigned to only the PA office, only that provider will have appointments and transactions in the PA location -- the NY provider will not have transactions or appointments in the PA location.
