Overview:
Denticon's Medical History document is divided into three sections: Medical Alerts, Dental Questionnaire, and Medical Questionnaire. This article covers how to manage Medical Alerts — including how to add, edit, delete, and reorder them — and how to control whether alerts are marked as required.
Medical History setup is global to your entire Denticon account (PGID). Any changes made here will apply across all office locations, not just a single site.
Managing Medical Alerts
Use the options below to add, edit, delete, and reorder alerts within the Medical Alerts section.

| Add a new alert | Click the Add New button, complete the alert details, and click Save. |
| Edit an existing alert | Select the desired alert, click Edit, make your changes, and click Save. |
| Delete an alert | Select the desired alert and click the Delete button. |
| Reorder alerts or headers | Drag and drop the alert or section header into the desired position. Headers can be moved the same way. |
| Save a draft | Click Save Draft to retain your changes without publishing them to patient records yet. |
Save Draft vs. Save — What's the Difference?
Save Draft preserves your in-progress changes in setup but does not publish them — patient-facing medical history forms will not reflect the updates yet. Save publishes the changes and makes them live for all new medical histories going forward.
Any changes made to Medical Alerts setup will only appear on new medical histories created after the change is saved. Existing patient medical history records will not be retroactively updated to reflect the new configuration.
Dental & Medical Questionnaires
The Dental Questionnaire and Medical Questionnaire sections work in the same way as Medical Alerts. Both support more varied answer types — including write-in boxes, checkboxes, date fields, and yes/no options — in addition to the standard yes/no format used in Medical Alerts.

The same Add, Edit, Delete, Drag-and-Drop, Save Draft, and Save functions apply to both questionnaires.
Removing the "Required" Status from a Medical Alert
When a Medical Alert is configured as required, a red asterisk (*) appears next to it, and staff cannot save a patient's medical history without addressing that field. If you need to remove that requirement, follow the steps below.
- Navigate to Medical Alerts
Click Setup in the top menu, select Medical Setup, then choose Medical Alerts. - Select the alert
Locate the alert marked as required (indicated by a red asterisk) and select it. - Click Edit
Click the Edit button to open the alert's settings. - Uncheck "Set State to Required"
Locate the Set State to Required checkbox and uncheck it. - Save
Click Save to apply the change. Repeat for each alert that needs to be updated.
