Ensure custom Phase Description is not saved/retained when all procedures are complete:

• When a user adds a new TX to a completed Phase, then the ‘Phase Description’ will be removed and a warning message will be displayed

• Given a phase is completed, when a TX is reactivated after pasting or deleting from the Ledger, then the ‘Phase Description’ will be removed

• When a ‘Phase Description’ has been removed, then users will be able to add a new phase description and it will be saved and displayed correctly

• When an item is updated to an Unaccepted status, then clear the ‘Billing Order' and 'Estimated Insurance' values for the procedure and re-estimate all the TX’s for the TID and PID

Improvements to 'Edit Tx/Phase’ modal behavior:

• Given a ‘Duration’ is being selected on the ‘Edit Tx/Phase’ modal, when clicking the dropdown, then the duration value will be started from the interval value which is saved in the “Setup > Office Setup“ screen

• When selecting a Phase from the Treatment Plan list while scheduling a Patient's appointment from Scheduler, then the chosen Phase's “Duration” and “Production Type” should be updated in the appointment's “Duration” and “Production Type” fields
