REQUIRED FIELDS

Patient and Responsible Party Phone Number Required
Patient Date of Birth Required
Patient and Responsible Party Email Required
SSN ( New Patient Registration Only)

Go to Setup / Account Info / Advanced Tab / Edit / See the Required Section.
