The Medical History document in Denticon is global to the PGID, meaning that all offices will share the same medical history document. Keep this in mind as the document is setup in order to ensure that all necessary information for each provider/specialty is taken into account.
- The medical history document is divided into three parts: Medical Alerts, Dental Questionnaire, and Medical Questionnaire.
- The Medical Alerts is a section that includes Yes or No questions regarding allergies and history of conditions.
- The Dental Questionnaire and Medical Questionnaire have more options for answers such as write-in boxes, checkboxes, or a date box.
To modify existing items in the medical alerts section:
- Use the two lines to the left of the item to move the order or placement.
- Note that all items will be listed in alphabetical order.
- Use the pencil icon to the right to make edits to the text, or click the red trashcan icon to delete the item.
The buttons on the lower right control adding new items to the list, as well as saving the medical alerts section.
- The first choice is the ‘Add New Header’, which will create a new section within the Medical Alerts page. The headers are separated here in blue, whereas the alerts themselves are listed below the blue headers.
- When adding a new medical alert, there is a checkbox that says ‘Flash Alert’ to the left of the alert itself. If the box is checked, a flash alert will be generated on the patient’s account if that item is selected ‘Yes’ when the medical history document is being filled out.
- Items with the two asterisks “ ** ” prior to the text indicate a flash alert will be generated when it is selected.
- Once any changes have been made to the document, make sure to click ‘Save as Draft’ or ‘Save’ in order for the changes to take effect.
- When selecting ‘Save as Draft’, the medical alerts page will retain the updated setup, but will not be published as the new medical alerts form until it is truly saved.
Medical Questionnaire
The Medical Questionnaire contains questions that may require more long form answers than just a yes or no answer, although that is an available choice for an answer in the medical questionnaire.
The same function applies here in the Medical Questionnaire as the Medical Alerts to adding a new header and modifying or deleting items.
- When adding a new question to the questionnaire, there are several new answer options available including Checkbox, Textbox, Date, Memo, Yes/No or PickList.
- A checkbox answer gives the user the option to check off a box if the question is applicable.
- A Textbox will provide a short answer box and the Memo option provides a longer answer box.
- The Date box will provide the user a place to select a specific date.
- A Yes/No option provides a dropdown to select Yes or No.
- A Pick List allows for a pre-defined list of answers that the user will be able to select from as the form is being filled out.
Dental Questionnaire
The Dental Questionnaire contains questions relating to the patient’s dental history. Just like the Medical Questionnaire, these questions may require different kinds of answer types.
The same function applies here in the Medical Questionnaire as the Medical Alerts to adding a new header and modifying or deleting items.
