If the insurance plan is an indemnity (aka: UCR-based) plan –
· the fee schedule used will be the office’s default fee schedule
· there is no attachment of a fee schedule to the plan
· there is no attachment of a fee schedule to the insurance carrier
If the insurance plan is a PPO (aka: contracted) plan –
· the contracted fee schedule is attached to the carrier
If the insurance plan is a Managed Care or Medicaid plan –
· the plan’s fee schedule is attached to the plan
NOTE: The illustrations assume the practice is a single location dental office. If the dental corporation has more than one location, the office-specific fee schedule relationships should be defined through the “Fee Schedule Assignment” screen.
