For a procedure involving a supernumerary tooth to be included on a pre-authorization claim:

· Enter the procedure code in the Treatment Plan, in the box to select tooth number click the drop down menu

· Click the button to show supernumerary teeth

· Edit the tooth number field

· Click the Save button

· Generate the pre-authorization claim
