Introduction:
This guide with walk you through the process of using Message Hub to send Consent Forms to Patients via SMS message and the Patient Portal. See Additional Information for a link to the article for emailing consents via Message Hub.*
Process description:
- Go to the Patient Overview.
- Click the cell phone number to access Message Hub.
- Click the Consent Request button.
- Select your Consent Forms.
- Press Send.
- The patient will enter their Zip Code and Last Name, and complete the forms.
- The forms are then saved under Patient > Notes.
Prerequisites:
- Security Permissions
- Patient - Email Or Text Message Full Control
- Reports - Custom Letter Full Control
- At least one Consent Form under Reports > Custom Letters > Patient Consent or Reports > Advanced Custom Letters > Patient Consent.*
- The Patient Portal must be setup.*
Step-by-step instructions:
- Select a patient from the Scheduler or Search Patient tool and proceed to their Patient Overview.
- Click the hyperlinked phone number to open Message Hub.
- To launch the selection module, click the Consent Request button. It looks like a person standing in front of a piece of paper.
- Choose your Consent Forms and press Select to confirm selections.
- Press Send to deliver the Patient Portal link to the patient's cell phone via SMS.
- The patient will enter the Portal ID and necessary information (Zip Code on file and Last Name) to access and sign them.
- The link will expire in 30 days.
- To find the completed version of the Consent Forms, click the Patient > Notes drop-down menu. The entries are saved with the data the patient provided here as an attached file.
Tips and best practices:
- The E-Consent link expires after 30 days. Make sure you communicate with your patient to get the form signed before it expires or you will need to resubmit the form.
- In cases where the provider wants to sign the consent form, the saved consent form may be printed from the Patient > Notes drop-down menu, signed by the provider, and scanned back into the Patient Notes by the office.
- If the office utilizes an in-office signature pad, the Patient and Dentist may sign the consent form(s) at the same time or while the patient is in the office.*
