Issue description:
Claims are sending for a patient with the wrong place of service but only for a single patient. All other patients send with the default place of service/place of treatment code setup under
Signs:
Patient received treatment for a place of service other than the default place of service on a previous claim. Each claims since has only had this place of service code.
Basic troubleshooting steps:
- Check the Default Place of Service code by going to Setup [dropdown menu] > Offices > Office Setup. Then select the home office of your patient > Advanced [tab] > Edit [button].
- This will display the present Default Place of Service code. It cannot be displayed without clicking the Edit [button].
- Select the Ledger of your patient.
- Find the 'Show Only' [dropdown menu] > select Only Claims.
- (The Clear [button] can be pressed when done to dismiss this selection.)
- Click the date of service [hyperlinked text] for each claim until you can find the first claim with the place of service code switched.
- You can review where the service was done via the Claim Fill-Out Information [button].
- For claims are can still be edited (press Unclose Claim [button] and make edits then press Save [button]), you can change the place of service code here.
- For dates of service following this, claim(s) may need to be adjusted and resubmitted after unclosing said claim(s).
- If this cannot be done, the claims may need to be voided/recreated and resubmitted.
- In most cases with patients seen at an unusual place of service, this can help to find the one claim that needs this code to remain
- (ex. patient is usually seen at an office but was seen only once at a hospital).

Before submitting any claims, always check the Place of Service/Treatment for the previous claim.
- The first time a claim is created, the form is assigned the default as per the office setup; however, a team member in office can change this.
- When a team member does change the Place of Service/Treatment on the Insurance Claim Fill-Out Form screen, the system remembers this.
- After this selection is made, each following claim is created with the same Place of Service/Treatment.
- You can still change this before creating the claim, then the following claims will remain with your selection until another team member makes a new selection.
