We want our patient to review the demographic information we have entered in Denticon. How are we able to generate a document for the patient to review?

Offices are able to provide a document to the patient so that demographic information may be reviewed and hand-corrected by the patient:

• Patient info – name, address, phone number(s), email address, birthdate, Social Security, Driver’s License
• Responsible Party info – name, address, phone number(s), email address, birthdate, Social Security, Driver’s License
• Primary (and Secondary) Insurance plan
• Medical Alerts
• Dental Questionnaire
• Medical Questionnaire
• Ability to have the patient sign the accuracy of the information

To generate the patient-specific document:

• Access the patient’s record
• Click the patient’s specific “print reports” icon
• Select the “Medical History” option in the drop-down list
• Indicate the desired parameters
  • Medical History date (the date for the most current medical history will automatically populate)
  • Print Patient Information
  • Print Responsible Party Information
  • Print Insurance Information
