The Insurance Billing Provider is defined in the Office Setup.
To check the current Insurance Billing Provider for the office:

Go to Setup>Offices>Office Setup

Select the office location on the left

The Insurance Billing Provider is indicated within the "Info" tab, "Other Info" section.
To change the current Insurance Billing Provider for the office:

Click the Edit Office button

Indicate the desired Insurance Billing Provider in the “Insurance Billing Provider” field

Click the Save button
To add another provider to the "Insurance Billing Provider" list, go to;
- Setup>Providers>Provider Setup
- Choose your provider on the left
- Click "Edit Provider"
- Check the box within the "Info" tab for "Billing Provider"
- Save
